Washington requires the covered application information to be recorded on the day the pesticide is applied and retained for at least seven years from that application. WSDA permits any recordkeeping format containing the required information, but can require submission on its official form. Choose the worksheet for the type of work, then organize the daily entry, the farm's copy, and the archive. [1][2]
Who falls within the Washington record rule?
RCW 17.21.100 covers certified applicators licensed under the chapter, people required to be licensed, people applying pesticides to more than one acre of agricultural land in a calendar year, public roadside applications within the stated scope, and specified landscape applications. The acreage provision is one route into the rule. A licensed applicator should assess the licensing route rather than treating acreage below that threshold as an automatic exemption. [1]
WSDA's guidance also identifies unlicensed landscape users at locations including schools, daycares, apartment complexes, shopping centers, golf courses, and parks. Separate these settings when choosing a form: a landscape-service worksheet is designed around a different work pattern from a production-field application record. [2]
Select a WSDA form by the job structure
The agency directory lists the following approved formats. Its English and Spanish links are grouped by version. Use the directory as the source for the form you actually need rather than relying on the filename of an old attachment. [2]
| Version | Directory description | Useful selection question |
|---|---|---|
| 1 | Single application, one location, one applicator | Are you documenting one distinct treatment? |
| 2 | Multiple applications, one location, one applicator | Does one person need several application entries? |
| 3 | Multiple applications, one location, one or more applicators | Must you distinguish the people performing several treatments? |
| 4 | Commercial landscape applications | Is the work a landscape service? |
| 5 | Commercial pest control operators | Is the work in that operator category? |
The directory supplies Spanish forms for versions 1 through 4. It also provides a form-approval request for a modified worksheet to be listed as an approved version. That approval process is distinct from the agency's general statement that records may be kept in any format containing the required information. [2]
Assemble the application fields
The statute's core list connects location, date and daily beginning and ending times, product name and EPA registration number where applicable, crop or site, amount per acre or other suitable measure, concentration, treatment size, licensed-applicator contact information, actual applicator identities and applicable license numbers, and wind direction and estimated velocity. It also permits other reasonable information required by rule. [1]
Use separate headings for rate, concentration, and treated size. Read a completed row as a description of the work: which product, at what concentration, at what amount per unit, over what actual area? A number copied into the wrong quantity column can look complete while describing a different measurement. Put units in the headings and retain any explanation needed to interpret a partial-field treatment.
The licensed applicator's name, address, and telephone number belong with the names of the people actually making the application and their license numbers when applicable. A company name alone does not express that entire identity field. Before repeating yesterday's entry, compare the actual team with the names carried forward. [1]
Wind information and the stated exception
Washington calls for the direction and estimated velocity of wind during the application. The statutory wind subsection excludes applications of baits in bait stations and applications inside structures. Keep the exception attached to that wind requirement; do not use it as an instruction to omit the application record itself. [1]
For a record review, compare the weather notation to the application times written in the same row. Explain any abbreviation your team uses for direction or speed. Preserve the original application note when resolving a transcription question. These are documentation suggestions and do not supply a product's weather limits or authorize an application.
Finish the entry on application day
The statutory recording deadline is the same day as application. Establish an internal handoff that brings the field note to the person finalizing the record before that day ends. Check daily beginning and ending times, especially where a treatment continues across dates. The statute expressly describes the times for each day the pesticide was applied. [1]
An editorial closeout checklist is to confirm location, product, crop or site, quantity meanings, actual people, daily times, and wind notation or its applicable exception. Then attach the supporting note and identify any corrected transcription. Keep the earlier wording legible in your correction history rather than silently replacing an unexplained number.
Give the agricultural customer a copy
A commercial pesticide applicator treating an agricultural crop or agricultural land must provide the owner, or the lessee when acting for that lessee, a copy of the required record. The customer copy does not have to be on a department-adopted form. Maintain a handover reference linking the copy to the treatment and the recipient. [1]
When the customer employs one or more employees within the statute's definition, the employer also retains the records for seven years. On the farm side, organize incoming contractor files by treatment location and application date. On the contractor side, preserve which completed version was delivered so later questions refer to the same information. [1]
Be ready for access and copy requests
Records must be readily accessible to the department for inspection. The statute lists several authorized recipients, including treating health care personnel and specified government bodies. When treating personnel need a record to determine treatment, copies are supplied immediately. For other requests under that subsection, copies are supplied within 72 hours. [1]
If treating health care personnel request telephone information, it must be made immediately available, followed by a copy within 24 hours. Identify a backup person who can locate the records and use the telephone retrieval route when the usual office contact is unavailable. This preparation should focus on document access rather than on collecting unnecessary medical details about the affected person. [1]
An archive another person can navigate
Washington includes inspection of these records as part of an on-site agricultural-land inspection under the chapter. The inspection checks whether the records are readily transferable to a department form and readily available to the people entitled to them. Test those tasks before an inspection: locate a past treatment, explain its location key, and map its columns to the requested form. [1]
Keep an archive index with year, application reference, customer, location, and attachment names. Open a saved export independently of the live system and confirm that the applicator identities, units, and daily times remain visible. Preserve the field map or naming key used during that season. These editorial checks make the statutory access and transfer questions concrete without adding an invented filing schedule.
Compare a custom worksheet with an official version
Use the WSDA form directory as the reference for this review. Its approval-request procedure addresses a modified form seeking listing as an approved version. Preserve the version description alongside your internal worksheet so a colleague can understand why it suits the job. [2]
During a transfer exercise, place the custom headings beside the official headings and describe every correspondence. Pay attention to a combined weather box, abbreviated credential details, and repeated treatments sharing a location header. Make the relationship explicit when the customer address differs from the treated property. Identify which row owns a handwritten addition, which page completes the entry, and where the original note is filed. An empty or ambiguous field should produce a precise question to the person who made the record, followed by a documented correction. This exercise directly tests the statute's concern with ready transfer to a department form. [1]
